You would be joining an innovative team, supporting a 7 day a week service, delivering often complex symptom control, emotional support and guidance around social concerns to our local community. Working as a part of an integrated team you would work closely within a broad MDT, including our medical team, AHPs and family support.

Our community team is multi-skilled and multi-professional, that means nurses, medics, healthcare assistants, spiritual care, social work and occupational therapy.  With a wide range of opportunities to develop specialist clinical interests and qualifications. 

The team works 7 days a week from 9:00am to 5:00pm.  We invest in continuous professional development from day one. Successful acquisition of addition skills and competencies are reflected in salary at the end of probation and as part of annual reviews.

• Registered General Nurse (Level 1) / Paramedic
• Evidence of Continuing Professional Development
• Palliative care experience is essential
• Proven post registration nursing / paramedic experience.
• Proven experience of working effectively in a team.
• Minimum of 3 years’ experience in palliative care/oncology.
• Proven experience of using IT systems.
• Good organisational skills.
• Knowledge of symptom control.
• Ability to perform assessment, planning, implementation and evaluation of care.
• Excellent written and verbal communication skills
• Demonstrate an ability to supervise and support others.
• Ability to work effectively in an emotionally demanding environment.
• Caring and empathetic approach
• Excellent listening and interpersonal skills
• Good team player who is willing to support others/learn new skills
• Ability to work with confidential sensitive information & understand boundaries and support others to work in this way
• Excellent judgement and good decision making skills
• Flexible working to defined shift patterns including weekend working.
