Healthcare Professionals - Referral Process
If you are a health professional you can refer to all forms of our care (IPU stay, outpatient review, home visit, social care assessment, Spring Centre health & wellbeing services).
How to refer to The Hospice of St Francis?
Please download our standard referral form above. You can also ring one of our clinical secretaries who can send one or email us at firstname.lastname@example.org to enquire about our services. If you are a health professional you can also ring us if you would like to discuss a referral with a member of our senior nursing or medical team.
Patients can be referred for a range of different services:
- Inpatient Unit admission
- Outpatient review by a Physiotherapist, Occupational Therapist or Doctor
- Support at Home from one of our Specialist Nurses or Doctors
- Assessment by our Supportive Care and Social Work Team
Referrals are prioritised for admission and discussed on a daily basis with the senior clinical team. All referrals are assessed by the Palliative Care Referral Centre which operates 9-5pm on weekdays and 10-2pm at weekends/Bank Holidays.
Once we have received a referral, we will make direct contact with the referrer to give an update on when a bed might be available.

Please note that whilst we are carrying out some essential building and maintenance work here at The Hospice of St Francis including replacement of our water pipes this July and August, referrals will continue as normal. Please do continue to refer patients to us via the Palliative Care Referral Centre (PCRC) in the normal way as detailed above.
