Target Audience:

Allied health professionals including rehabilitation assistants and all professional staff working within a Hospice



  • Understand the important role AHP's have in advance care planning (ACP)
  • Describe the benefits and challenges of advance care planning
  • Identify some different documentation available
  • Identify the ACP process and potential triggers for practice
  • Display awareness of the principles of the MCA to ACP process
  • Apply knowledge from workshop and case to own clinical practice
