We have a team of qualified and experienced social workers offering psychosocial and emotional support and assessing social and future care needs for patients, their families/ carers.

Social Care Assessment

We can help you to plan care, at either home or in a nursing home, exploring the different funding options open to you. Liaising with statutory and voluntary services such as Health and Social Care, Age U.K. forms a key role for this team.

Family Work

The social and emotional impact of ill health can affect the whole family. The social work team assist families in various ways including; help to navigate the care system, advocacy in more complex situations and if required mediation support within the family.

We are able to direct patients and their families to the appropriate support service to ensure that they are receiving the correct advice and support in claiming statutory welfare benefits and legal advice for applying for Lasting Power of Attorney and making Wills.


A carer is anyone who cares, unpaid, for a family member or friend who due to illness or disability cannot cope without their support. If required the social work team can meet with carers to identify their support needs and ensure they are linked in with appropriate carer support services.


Social workers have a duty to protect those who are vulnerable and at risk. Ensuring the health and welfare of those using our services is paramount. If there are concerns they will be addressed appropriately.
