Principles, challenges and benefits of advance care planning; significance of legislation of Mental Capacity Act; definitions; process and documentation used in advance care planning (e.g. clarification of advance statement, Advance Decision to Refuse Treatment and Lasting Power of Attorney, roles and responsibility of documentation, DNACPR process and documentation.  Advance Care Planning (ACP) in practice scenarios
