Referrals can be made by any healthcare professional within the Hospice's Catchment Area. This will usually be the patient’s GP, palliative care nurse or hospital team.
We encourage the use of the standard referral form which health care professionals can access here, or they can ring one of our clinical secretaries who can send one. We also have a special, confidential email address that can be used: firstname.lastname@example.org
Health care professionals may also ring us if they would like to discuss a referral with a member of the senior nursing or medical team.
Patients can be referred for a range of different services:
- Inpatient Unit admission
- Outpatient review by a physiotherapist, occupational therapist or doctor
- Home visit by a doctor
- Support at home by one of our Specialist Nurses
- Assessment by our Supportive Care and Social Work Team
Referrals are prioritised for admission and discussed on a daily basis with the senior clinical team.
We will make direct contact with the referrer to give them an update on when a bed might be available.
