Referrals can be made by any health care professional in our catchment area. This will usually be the patient’s GP, district nurse, palliative care nurse or hospital team.
We encourage the use of the standard referral form which health care professionals can download, or they can ring one of our clinical secretaries who will send one.
Health care professionals may also ring us if they would like to discuss a referral with a member of the senior nursing or medical team.
Patients can be referred for a range of different services:
- Inpatient Unit admission
- Day Hospice attendance
- Outpatient review by a physiotherapist, occupational therapist or doctor
- Home visit by a doctor
- Support at home by one of our six Specialist Nurses
- Assessment by our Supportive Care and Social Work Team
Referrals are prioritised for admission and discussed on a daily basis with the senior clinical team.
We will make direct contact with the referrer to give them an update on when a bed might be available.
